1. Obtain copies of relevant laboratory reports.

2. Fill out the MDPH's 'Lyme Disease Case Report Form' (attach laboratory results).

3. Send the completed case report form, with the attached laboratory reports, to the MDPH Bureau of Communicable Disease Control, Office of Integrated Surveillance and Informatics Services (ISIS).
