This module focuses on primary care, and it will also include discussions of the Patient-Centered Medical Home (PCMH) and Accountable Care Organizations (ACO).

- Define Accountable Care Organization as described in the ACA. Describe the Shared Savings method of payment for services.
- Describe challenges to the physician workforce with emphasis on supply of primary care physicians.
- Identify and describe key elements of the Patient Centered Medical Home.
- Define and differentiate between ACO, HMO, PCMH, and PPO.
